Which kind of form should I build?#
One builder makes all three. Pick the type when you create it under Start a new one, and Agent Studio fills in a starter set of questions you edit from there.
| Type | Use it to | What makes it different |
|---|---|---|
| Form | Capture contact requests, quote requests, sign-ups | One page, fast to finish |
| Survey | Qualify leads or gather feedback in steps | Multiple steps with a progress flow |
| Quiz | Score a lead or recommend a package | Points per answer, scored result screens and an answer key |
How do I build and publish a lead form?#
Add questions from 13 field types, map each answer to the contact, then publish. A draft shows visitors a closed notice, so nothing half-built goes live.

How do I send form leads straight to my booking calendar?#
Pick a calendar under Send visitors to book on and every visitor lands on that booking page the moment they submit, with their name, email and phone already filled in. You keep the lead even when they never pick a time.
- The Form or survey submitted trigger carries the booking link, so a workflow emails it to anyone who leaves without booking.
- The ready-made Follow up leads who did not book workflow checks for a booking after an hour, emails the link, checks again over the next days, and adds a call task if they still have not booked.
- If the calendar is paused, visitors see your thank-you message or redirect instead of a dead end.
Can my form change based on earlier answers?#
Yes. Give any field a show-when rule (equals, does not equal or contains) and it appears when an earlier answer matches. Hidden values fill from the link, so ?campaign=spring on the URL saves the campaign with the lead.
Quizzes add points per option, an answer key for right answers, and Quiz results ranges that show a different title and message for each score band. Score high on readiness and the result screen pitches your premium package.
Where do form responses go?#
Every response lands on the form's Responses page, newest first, with Export CSV for spreadsheets. It also fires Form or survey submitted (or Quiz completed) for your automations.
Collecting leads in another tool? Inbound webhooks on the same Forms page give you a private address. Send JSON to it, map the fields once, and each delivery writes the contact and fires Inbound webhook received.
Send every form lead to a booking page with Calendars, and embed forms on the pages you build with Websites.
